Hi, this is my entry for your contest. Please do let me know what you think. I've filled the pages with sample text, just to show how it will look once all filled in. It's done in Canva. I figured it would be easier for you to edit and generate a pdf that way. (Canva is free) and you can customise any way you would want to as well. Hope you like it!
