I want a Excel based or standalone program. This requires users to login and fill out the form with approximately 50 set of questions. I have attached the blank form that includes the set of questions. The users will use this form to make entries and want them to use the same to update the data already available on database. I've taken on a project developed by somebody else. It has a page that makes use of a vue file, where there are some fields to select parameters for a chart that is rendered based on the provided parameters. I've managed to add an additional dropdown but when the form is submitted, the new dropdown's selection does not get submitted with the rest of the GET variables to the code that renders the report. If you know vue and laravel and nova, you will probably be able to spot the issue and help me correct it quickly. I suspect there must be a place where I need to specify the field name and tell the system to send it along with the another variables, but I am not sure what the correct way is to do this, and where to do this.

I want to increase the conversion rate of my web form but don't know how to use autofill on my web form. Users should be able to auto-fill and I'm having a hard time getting this to work on mobile.
