Create Behavioral & Personality Surveys - Very Challenging!!

We need someone that knows how to put together a survey that will pinpoint behavior and personality characteristics in people. The survey should have about 25 to 30 questions. Once the survey taker has answered the questions, there should be a mathematical process used to plot the behavior on a grid that we have created. The grid contains 4 quadrants that has another 4 quadrants and there are 128 different cells on the entire grid. Each cell has four adjectives that describe behavior on business and personal levels (1 positive, 1 negative) for each.
If you can create such a survey, here is another twist. We wish to have our grid on an X/Y plot, but inside of an Octagon. In order to accomplish any of this, you must submit to us the approach you will take and have some history of creating such solutions. Because this is an extremely important project for us, we will need to check your work history and make sure you can deliver on time and on budget. Any generic "we can do this project" responses, will lessen your chances of being awarded the project. We really need you to be as specific as can about what you understand on this project AND how you might bring about the solution we are looking for.
