A webpage with a web service - repost

I need an ASP webservice written that is very simple. It will pass some name data and return a result. The webservice will be publicly available and likely to be deployed on Azure. I also need a simple webpage created with some basic css. The webpage needs to point to the above web service and provide the answer from the webservice. Access to the webpage will be via a logon, which will likely use an existing web service we have, although we may need a new web service for this as well. I need this done fairly urgently. I am using this small project as a test for a lot more potential work.
