Basic Webpage Design

I need a pretty basic page created. I have uploaded a hand drawn mock-up to help understand my thinking.
So what I need is:
1. The site should be written as HTML5.
2. I want it to work on mobile devices. Probably using bootstrap.
3. There is a testimonial section on the right hand side that should have comments and star ratings. We need the ability to add a testimonial
4. There is a Video section that I want to play a video that I will upload. I want to use a Video player that will work well on mobile devices. Happy to encode it in any format required.
5. The video should be playable from a button below it, that I can disable. I want people to pay a small fee before playing the video. I am not sure the best way to do this, and am happy for options.
6. The right hand section (History) is just text. I want to include a couple of paragraphs about the history of the video.
7. The small bit next to the 99c that says "Hover" Payment History should display a window about the Payment Method, namly Paypal and that it except credit cards even if you don't have an account. This should be a hover window over an icon.
8. Where I have put the text 8c, 2c etc is what I though were the bootstrap column sizes. It isn't text and I also don't know if these are correct.
The main thing to consider is that the video is the star of the show. It needs to render properly on a mobile device and also to go full screen on a mobile when you push play. Thanks all.
I just need the HTML, Javascript and CSS delivered to me. I will do the back end work and get it onto the web. I would also like some design work done around the colours and look and feel etc.
