Convert fax document to editable PDF

Hello, I need someone to recreate an editable PDF file from a faxed document. I've included the document, logo, and example form. Note: I need each page to have the logo, phone number, address, website url in the header. Each page should be numbered, down in the footer. This form needs to be editable and have an email button and submit button at the end of the last page.
