Create Excel User Form to capture data for business cards

I need a simple Excel spreadsheet with a data input form. The idea is that users can fill in all the personal information they need printed in their business cards. Then the data would be stored in the spreadsheet. After data, we could optionally extract the same data in the order we need it to be applied to the business card layout in a txt file, for example. The input form should have the fileds you can see in the attached file.
