Build an accordion form

I currently have a form (see attached) built but I need the page redesigned. Currently the form is a three page form, but when the user clicks submit the next form page populates on the SAME screen. I want the form to keep the same functions but I just need the page redesigned. See the attached image for reference. I will provide the background image (like the attached [login to view URL]). Again the form will have the same form functions but I also need the input fields to look like the example (white over black background). I hope this is clear, if not please ask for clarification? Also, I need this completed as soon as possible so if you can complete soon please say so when bidding. Again, if you need any clarification please ask? Thanks.
