Combine existing design (input PSD file) and existing newsletter subscription form (input JAVASCRIPT code)

Dear all, we need somebody who combines an existing design (PSD) with an existing newsletter subscription box (JAVASCRIPT code). The deliverable is a new designed newsletter subscription box, we want to integrate to our website. It should: - look (100% exactly) like the existing design (PSD file) - work (100% exactly) like the existing newsletter subscription box (javascript code) That's it!
