Hi there, I have developed similar project for market research companies in Canada and USA. Here are the features that I can build - - Prepare the questionnaire / Load the questionnaire using CSV file. - Prepare the data map - the Question IDs and the options. - The questions could be closed end, open ends or with multi select options. - Presenting the questions based on previous questions. - A responsive web design so that it can be used easily on mobile phones as well. - The data will be collected in database for download in CSV format. - It would be an independent application which you can embed in any website. - The application will automatically create a unique ID for each respondent so that you just embed the URL. To add further in this, I can also design and develop a dashboard for data analysis. It would be a visual data representation in bar/columns/lines/pie charts.
