Design new order document with changes in Indesign

At the moment we use an order document we fill in if an order is comming in. We need some changes on it and maybe a new design for it. We need the document in Indesign so we can change thing after the job is finished. It must been in a A4 format and may be on two sides. Urgent job so please only write a reaction if you can start right now !!! Attachment: scan1 is the document we use at the moment, the new design must contains same this. Scan 1 2 en scan 1 1 are the new item that must been on the new document too.
