I'm looking for a freelancer, that can develop a web form to calculate some fields and save those fields into a database table. Sample codes and a video can be found in attachments. I'm expecting 3 hours of work. Please review carefully and send me estimated completion time. I'll accept the work only if code works and results are approved by my supervisor. There will be future opportunities for successfull candidates.
