Dear Client, I am interested in your MS-Access user interface design project. I would like to purpose build this system with custom Ribbon Menu and list based records system. Furthermore, the main data entry/edit form will have clear text as well as logical clustering on the information. First user will create the profile of the patient, when user click on the patient menu, the list of existing patient will appear, user will click on ADD button to add a new patient information or click on edit button to edit the selected patient information. Now, the user will go to on the list of Treatment Facts, I hope there will one or more Treatment Fact for each patient, As you have linked Treatment Follow Up table to Treatment Fact, is there one to one relation (ie is for single records of Treatment Fact will only have one record of Treatment Follow Up table)?
