ASP, MySQL, Ajax Forms Data Display(repost)

I am seeking a reliable, skilled programmer who has good communication and good rating. Must be skilled in **ASP**, **MySql** database programming, **Ajax** and good? with logical structure for user experience. This is adding programming to a current site for user areas to enter information in forms and functions and options, as well as admin area functionality to edit/change user info or applicable information. **Attached is a document detailing the work for each page file.** This is one phase in several as we expand the site. Please do not bid if you do not have experience in these areas. **I will also provide bonus on top of payment for good consistent communication and follow through on deliverables**.
