ASP.NET Web Application

I need an ASP.NET Web Application in C# and SQL Server DB (or MS Access via ADO). Use schemas (XSD) for style definitions. It would prefer if you put all DB access and calculations logic into a DLL. Instead of ASP. Feel free to use VB for DLLs if you prefer so. Feel free to hard code options into the DLL. I am mostly interested in the web presentation part. More details are in the attached Word file and drawing. Layout for Print2 report is to be given later, but there is nothing special there. I need basic functionality of the main form to be completed within 1 week.
