Dim screen popup form

Looking to add a timed popup function added to one of our pages. After a preset amount of time say 5 seconds, I would like the existing page to dim and a popup form to appear asking for name and e-mail address. There will be 2 buttons; submit and cancel. Pressing cancel will return to the underling page pressing submit will generate en e-mail message to two administrator e-mail addresses. The admin e-mail adddresses should be stored in an external inc or xml file for easy editing. I have attached sample code from my existing page. It is quite simple but uses a combination if css divs and tables. So far I have been unable to create a popup that works correctly with the sample page. Must work correctly on IE 6 & 7, FireFox, and Safari browsers. Code can be css & javascrip or .net. E-mail generation can be .net generateed or php. Installation instructions will be required. ## Deliverables Sample html and css pages are provided.
