Create custom phpBB theme and setup categories.

I require a custom theme to be developed and installed for a phpBB forum. The forum is already installed but has no categories in place, I would also like these setup with specified permissions (see attachement). The theme will mirror supporting website [[login to view URL]][1] (see attachement) in terms of colour scheme and heading. I would like the header of the forum to look identical to supporting site including same logo and navigation buttons. The forum will also be the same width as supporting site.
