contact page done for website

I need a contact page done for the website savealotroanoke(dot)com. I would like some special effects added, like a contact form that lifts up from the contact us tab (see website) and users can quickly fill out the form and it returns back to normal position. It's just an idea :) I think that requires javascript or ajax. I need this done today, to at least get the contact page up there. Best ideas will probably get the bid. Should be a simple task for skilled coders. ## Deliverables i will give ftp access to the winning bidder.
