Award Application develop in PhoneGap

Hello to everyone, we search for one phonegap developer that can make the following. One application multiple mobile os (iOS, Android, Windows), as we know with phonegap you can do that. The idea of application is like survey. We make singer awards this summer and we need to make phone app for that. So the idea is 10 pages like survey, every page has to select one answer to our question (please see the attachment "[login to view URL]"). When the people select an answer then go to the next pages (click button "Next") which is different questions with different answers until to go at the end and then enter the contact details... Also we need it to can make changes anytime through admin panel on the web and also we need the ability to send notifications anytime we want. Please is important to know that we want to upload the applications and to the stores (we have the needed development accounts )
