Build 2 Simple Contact forms with php and jquery

I have an html website that needs two types of contact-us form.
Form 1: they can select in drop down box - type i.e. jobs, partnerships, investors etc...submit
Form 2:
When user clicks on this one... it goes to page where they are asked 4 questions. If they answer all 4 questions properly.. they can submit information... if one of the questions is not answered properly...they get a message "Sorry, based on your responses, we are not a good fit for your product"
I prefer to build form in php and jquery/javascript. Error checking is mandatory.
