Hey

I currently have a booking form: <[url removed, login to view]>

Firstly i want it to use JQuery UI for the layout.

Too look like this: <[url removed, login to view]> (The login Box)

Read more below, for how each panel should display, All functionality is already in place, you only have to add additonal items and price panel.

Thanks!

## Deliverables

I currently have a booking form: <[url removed, login to view]>

Firstly i want it to use JQuery UI for the layout.

Too look like this: <[url removed, login to view]> (The login Box)

The buttons would change too.

The data picker would change style as well.

Please look here for how the form should look and function like: <[url removed, login to view]>

Select any date, and any postcode, then click ok.

The tables should be the same width, but not centered on page.

The margin should be 25 left, top and right.

**Panel Info**

Panel 1

Item (Selection Box as before)

Date (JQuery UI)

Drop Off ... Collection ...

Additional Items (see how the other one works) they click a checkbox and lists all items where they could add more items to the order.

Panel 2 - "Your Information" and "Other Information" - On same page

Panel 3 - Payment

Panel 4 - Confirmation

Panel 5 - **REMOVE**

To the right of this panels i want the little box they have , that shows pricing and date that updates when the client adds more products or changes delivery or select overnight.

This should be easy since every thing is done, just changing the layout to jquery UI, you only need to add the little pricing box.

I dont want it an overlay anymore, so no button and should show page load.

You are to ensure that it is functioning like it is now! Capcha, Required Fields Etc!

Finally... i would be calling this form though Fancy Box, this would load up an IFRAME - There is currently a var in the current JS that get a value (select_item) when clicked. I want an additional one called hash.

So my URL to link to this form would be like : [url removed, login to view]

The hash could be a echo $_GET['hash']; Show the hash at the top of the form for testing purpose.

You also need to ensure the style (css) sticks! This does not have an header or footer its just the form.

You are NOT! connecting this to a database, your just working on functionality and style!

Thanks!
