Copy Form into AJAX

Copy form with 100+ options into Ajax/json/xml. The form is in the specified financial software. See attachment for the link to that software and a screenshot of the form. The ajax form should function like the one in the software. Building the form would involve 1. Copying all the options and values into ajax/xml 2.
