Custom Page/Plugin Wordpress front-end posting

Hello, I need a Custom Page or a simple plugin that helps me or anyother logged user to post from the front-end. The post will include some extra fields: Name: Position: Ceased SInce: Reasons: Image attach: Every logged user (with the abilitty of write a post) will see the form and can post a new registry. Every registry can or can't include an image. (if doesn't have will show a 'no Image' Image) Every logged user need to be able to delete it's own post. Admin will be able to delete any user posts if needed. All of this must be available from Front-End. The plugin or the page must have a way to change the category ID where the post will be saved and to add or remove special fields, its ok if it's needed to hardcode the changes or by a simple admin panel, but you need to show me how. You can use anything you want, just keep in mind that some javascript is used in the current website and javascript conflicts should be resolved by you.
