I need to have a landing page, thank you page and matching email template designed and coded. The landing page will have a contact and survey form that needs to be coded in javascript. I will provide the organization's logo and a single graphic; you will design, code and script the final package so that I can install it on the organization's LAMP server. I need to have the finished files in hand by 5:00 pm EST, 11/16. Bonus for early completion. Please do not bid if you can't complete this project by this time. Please use PMB to show examples of your design and coding work. I will not award the project without them.
