Case management records - a simple web application

Build a small, simple case management application. MySQL DB, PHP,
a handful of form pages/scripts to deal with:
1. records of "cases" (or "jobs"), including blog-style notes/text updates
2. records of contacts
3. records of related "items" - document attachments, URL's, photos etc
contacts, items, dates etc comprise a "case" record.
No layout work, just integrating new html forms into existing html layout.

create user/pw
log on with credentials
create new Case
assign contacts
assign dates
select Case to open (sort/filter, open vs closed)
edit case
add text entries (dated, user)
attach "items - photos, documents
print/export Case record
mark case as closed
