Add a ''type word seen in picture'' feature to a website form

This project has two tasks. The first task is the main part. The second task is just an additional piece of work that I need done. First and foremost,? I own the website [[login to view URL]][1]? and this website has a form where people submit questions. Recently, I am getting spammed all the time. So I would like to add one of those picture words that users enter when they submit something on the form so that I don't get spammed. This feature must be: - Attractive - Allow me to preselect 5-10 words - Match the colour scheme of the site - Not disturb any of the other features of the site. Second, Please consult the attached pictures. You will notice that in my admin panel, the database of users who submitted questions is cutoff graphically (due to a recent re-design). You will fix this.
