Build Single Webpage w/ .PHP Script

We have a simple .php script (attached to this bid post as a .zip) which scrapes online directories for contact information of prospects and auto-generates a .csv file. Right now we have to edit the script from within the code and change the search criteria in order to use the script for various criteria. We are looking for a coder to create a php-based online form which will allow us to input the url we want to scrape and the location to save the file(s). The form will incorporate the attached scripts and perform the tasks when submitted. Also, it seems that the script is producing duplicate entried. We would like this to be corrected as well.
