Camp Registration Module

We are looking to upgrade our online signup process for our youth sports camps. We need an application that can handle these requirements: * Take all of the necessary information from our customers (normally parents will be signing up the kids so needs parental as well as child information ... possibly multiple children) * Save it in their own personal account for future use (so the next time they want to signup they don't have to fill in all of the information again) * Can take payments (thru PayPal) based on different options chosen by the customer (such as: rent equipment, early signup discount, other discounts, etc) * Auto sign a medical waiver (clicking an "I agree" checkbox is acceptable, as long as there is a link to the waiver for them to read) * Email information automatically goes into our email management system (Vertical Response is the vendor - again, the "I agree" checkbox is acceptable for this... would actually like it required) * Allows modifications/additions to fields * A management console that allows mgmt to view current signups, modify any data, etc. * Can handle camps for different sports and different prices for each camp if needed. Here is what our current signup process looks like: <[login to view URL]> It works OK, but no longer fits all of the information we need to gather, as well as all of the functionality we've stated above. I would like to also be able to resell this application to other teams, schools, etc. who have expressed interest to me in a signup module they can use for their teams/organizations.
