customize DT regiser component

i looking someone implement for me a few features. there are what i want: I've done a few modifications and I basically need to have an area for our members to be able to check off a list of events they attended and it printouts out a report. I can supply the report which is a basic one page layout. The stipulation is I need this report to look almost exactly like it. My thought process is they simply place a check box beside the events they want on the report. It takes them to the next step which verifies any information that is needed and is pulled from their profile. Then it simply prints it either to the screen or a pdf format. i will provide more detail for winner.
