Coming soon landing page with a beta invitation system

For this, we want to create a coming soon page with a form to sign up for a beta invite. However, we want to create a viral beta invite system. How does it work? It’s actually fairly simple: after you sign up, we provide a custom, unique short URL that we encourage you to share out via email, social networks, etc. When somebody visits that URL and signs up, we can track it back to you and give you credit for it. After 4 people sign up with your link, you make our “priority access list” and we let you know via email. So I need to have a small back end system where we can track a person along with how many people they have signed up under them. It would look like something similar to the attached file. Also, please look at [login to view URL], once you sign up they then tell you about how you can invite your friends.
