Affiliate Site and Traffic Wave Integration

I have a membership site, [login to view URL] , and would like to add the abiblity to create splash pages with the following properties. 1. They should include the header and footer of the main site, 2. They should have a way to add content in the admin area, much like the "custom pages" area. 3. They should behave much like a banner or login ad, with an affiliate link unique to each member. 4. They should have a traffic wave capture form with 3 fields. Name, email, and sponsorID. The sponsor ID field should be pre filled out with the sponsor's The 10K Challenge ID, and should be hidden (field not visible on the page). This way I can add links to the main site in the emails, and they will be directed to the sponsor's affiliate link. I have already created a campaign in traffic wave for you to modify. I will need both the subscription URL and confirmation URL to redirect to the sponsor's unique affiliate page. (hoping this is possible).
