Car insurance data form (Write some Software)

I would like a car insurance quote form for UK that includes the use of the vehicle finder widget which works when the car number plate is entered . The form will be customisable ( add extra data boxes if needed later on ) and include google analytics's . The form will also match in with my existing website design and will collect all the data provided in this 4 page example [login to view URL] and have a car registration search box the same as the page where this link lands .
