Convert Indesign letter size brochure to web page

We would like to have someone convert the attached PDF file, (which will be supplied in the original InDesign format) and merge it into the following page with all the links at the left and bottom intact. [login to view URL] The text and picture of the journal will go away and be replaced with the content in the PDF file. We do not want an image based version, we want a real html version. Assume pictures are in either the image folder or the root folder. All code should go back into the one HTML file listed above. Thanks.
