Hello i have a flash image i would like to create. This flash image will be an image of a human body part where users can select parts of body and write a description of what part is hurting, after user selects the part they will continue to write down information like email best time doctor to call them, etc. I have included an image of how i want body image to look to the user. I will need a form embedded in left center part of my website and form to work within the wordpress theme. thanks happy bidding.
