Data Collection online forms

I have several forms that were made in publisher. They are currently are printed and filled out by clients on paper. We would like to collect the data online. I need to host these forms on my website. The forms would need to become interactive, collection of all data would be emailed to our service center for the next 2 months, and then hosted on our servers. I would like to see examples of similar work. I would like a developer who can finish the task ontime. I will provide the first set of forms to be converted. If I like the work, price, and speed of completion, I will award the remaining 18 projects to the same developer. Please email with questions.
