Design of forms linked to MySQL

The project you are bidding on is to develop 3 pages within a website with forms, and a MySQL database linked to the forms. This is for a company website based in Canada. We are looking for a freelancer who will work with us to create these 3 pages. Project scope includes the following: In page one three text examples must be shown in such a way that will be counted as text in a searches, on mouseover of the sample that text should enlarge, and stay enlarged until another sample is moused-over. On the same page, parallel to the samples will be an input text box where the user can copy and paste the sample text if desired. A continue button is required to advance the page. A if else checkbox is required, the use cannot proceed without checking one option. On the second page the user should be able to view the text they input in the previous form, ie preview/post. Both back and continue interfaces are needed on this page. Also needed are two checkboxes, linked to the database. On the third page a name and address form is required and must be linked to the database that holds the previous input data. Both back and continue interfaces are needed on this page. Also a checkbox is required, if not checked a message must appear, and the page cannot continue unless the checkbox is accepted. In addition to this, another checkbox is needed and must be linked to the same file in the database. All pages must use the same theme as the website currently uses. And all backend work must be linked on MySQL database. Please mention estimated time for project. Payment will be issued through PayPal when the website is up and running and fully functional and tested. There will be no payment upfront. Disclosure of full code is a must. A confidential disclosure agreement (CDA) must be signed upfront.
