Adobe Premiere DVD Menu Template

I need an attractive DVD menu template designed for adobe premiere elements version 3.0. This can easily be done in photoshop. The DVD I am creating is a set of video clips for medical students showing them how to do common clinical tasks properly (how to examine patients correctly demonstrated on model patients) hence the theme of the Menu system has to be medically related so for example a picture of lets say a stethoscope or syringe or BP monitor (just examples, use your imagination!) will be relevant to the medical theme. The template has to be visually appealing. The Scene menu should show 6 clips per page. Also I want a logo/title of the DVD to be integrated in the Menu Template that you design. The Title of the DVD is "Eazy OSCEz" (OSCE is short for objective structured clinical examination). Again giving a medical touch to the title would be ideal. Please show examples of previous graphic work you have done upon bidding. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
