C# forms application, MS-SQL

I need a c# forms application (not a web app) that can keep track of patient information and medical records. The application should use Microsoft SQL Server 2005 and C#. I need this to be written in Visual Studio .NET 2005. I have attached two word documents that detail what would be included on the forms and in the database. I would need you to create database tables based on the information in the two word documents, and provide forms that display this data. For anyone who has experience working with C# forms applications, this should not be very difficult. How you wish to layout the database and forms is upto you but you must stick to the following guidelines: 1. When connecting controls to database, you must use the BindingSources, TableAdapters, DataSet, etc. that Visual Studio .NET 2005 provides. I'm not accepting code where controls are manually tied to database through code (see attachment DB connection [login to view URL]). This means you will have XSD files, datatables, etc.. in your solution explorer. Whether you wish to tie to table columns, or stored procedure columns is upto you. 2. To jump from record to record, you must use the BindingNavigator ([login to view URL]). If you feel it is more convenient, then you can use TabControl to display several tabs in a form. (See attached jpeg image: BindingNavigator and tabControl [login to view URL]). I can't think of anything else important right now. If you are unsure on some of the requirements...like if two items should have a 1-1 or 1-many relationship, you can either ask me or take an educated guess...if i find something isn't correct I can correct it myself later. It would help if you can include comments so I can update the application if I need to. If you are going to create stored procedures, views, functions, etc...then I need the sql create scripts for those also. Thanks in advance. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). Remember, I need the whole solution including all code files, and you can include bin/obj directory also.
