Control Burn Database

We are looking for a Form that People will fill out this form will contain the following information: Address Fire Station (This will be a drop down box of all Fire Stations) Caller's Name, Phone Numbers, Start Date and End Date with ID Number. Now this will be submitted and under the page the information will show up in a table with an additional two options to Edit which will bring up a box, and Delete.
