Create PHP script to send .ics files.

Looking for a coder able to write me a small PHP-based form, that generates custom ICS files and mails the files. The form should contain: - Firstname / Lastname - Emailaddress - Select date + hour in future - When customer has selected and entered, email should be send with confirmation and ICS attached. When customer has selected date, the date should be logged and cannot be selected anymore. PHP file attached.
