car rental rates checker

Need EXACT functionality clone of carrentalhawaii(dot)com, but using my data. I do not require website design work. Only the rates checker from the above mentioned website. Customer inputs the following data, 1) Pick up location, Date, Time 2) Drop off location, Date, Time 3) Customer originating country 4) Vehicle Type Results page will return 1) Rates from 4 different rental companies 2) Include taxes and addons in brackets next to base rate 3) Form where customer will enter their personal info 4) This form needs to be submitted to an email address with the personal info and car rental info Project needs to be completed by end of sept or early oct latest.
