Appointment System for Doctors Website

I am creating a website for a local doctor here in Miami, FL. I would like an appointment system incorporated into the website. This system will be very simple, and must be done within 3-4 days. Here are the features it must have: -Calendar for both employees (admin backend) and potential patients (users) -Patient will be able to make an appointment (without logging in) and see the interactive calendar of open/available appointments -There will be two parts to this system, the user form and the administrative backend The User Form & Calendar: -Patient Sees a calendar to the right and there is a form to the left of the calendar -They select a date on the calendar by clicking it - THey are asked a series of questions, the first being "Are you a new patient?" -Once submitted the form will validate information and send them to a thank you page Administrative backend: -Employees and the webmaster will be able to login to the administrative control panel and do the following: -Add an appointment -Edit an Appointment -Remove an appointment -Set vacation time -Black a certain time frame (dates) on the calendar so that people cannot make appointments -Print appointments (based on a date range) -Export appointments to excel (this is optional) This needs to be done ASAP as we are running behind quite a bit and I would like to see this complete by the beginning of next week.
