I have an old html page with simple fields that contain just a username and password textfield and a login button. I need to revamp this entire page. See the old page at attached file old.jpg. My specifications are as follows: Need the new template to have the following: 1) Header div - Contains the logo 2) Content div - Contain a login panel, with the username, password and submit button - Contain a news panel with simple text and links - Contain a Join Us sub div for doctors - Contain a Join Us sub div for Clinic Assistants - (Optional) Contain a javascript .jpg changer? Just to 3) Footer div - Contains the copyrights text 4) Need my current logo integrated into the new template See attached file [login to view URL] for a rough example. I will prefer the service provider to provide design on image file first. Upon confirmation, then proceed to design it on HTML. Prefer the HTML to be workable on IE8+/FireFox/Chrome/Safari. Where possible, please attach best 3 design portfolio in your PM.
