build a counter application as a website

I need a simple interactive website as shown in the screenshot. It is used for a counter application for a tabletop game: the user can select the number of players and then for each player a score can be added, which will be summed. It has to work on desktop systems and smartphones. Please read carefully the detailed description below. Bid only, if you understand all requirements and if you can do it for the price you bid. - a single page website with HTML and JavaScript, no server-side scripts or server database - initially there are two columns, with "1" and "2" in the top text fields, an empty list and 0 in the sum below the list - if the user clicks in the text field, the name can be changed. It is a normal HTML textfield, so for a desktop browser the user can just type the name, and for a smartphone the usual keyboard pops up. - if the user clicks the "+" button, it is possible to increase the number of columns up to 5. With the "-" button a column is removed. 2 columns is the minimum number of columns. When there are 2 columns, the minus button is disabled. When there are 5 columns, the plus button is disabled. - if the user clicks the "Reset" button, a standard dialog is shown "Are you sure to reset the score?", with Yes/No option. If the user clicks "Yes", then all lists are emptied and all sums are updated to 0, and the dialog is closed. If the user clicks "No", then the dialog is just closed and nothing happens. - if the user clicks on a list, the list is selected (background color should change or something like this) and then the user can click on the buttons "1".."5" or the "Remove" button. If the user clicks on one of the number buttons, the number is added to the list and the sum is updated. If the user clicks on the Remove button, the last entry in the list is removed. - on a desktop system, the lists can be scrolled as usual with the vertical scroll bar, on a smartphone by dragging in the list - on a smartphone, the web application has to be always be full screen, and the columns has to adjust the width equally to fit the full width. The height of the title, name fields, sum fields and buttons are fixed, but the height of the list have to scale to fit the full height - on a desktop browser the content of the website should fill the whole browser window and should scale dynamically, when the browser window is resized. Same rules for fitting the elements apply as for the smartphone page - when the browser is closed and the webpage is opened again, the last view has to be displayed (stored locally), which means all names, number of players, numbers in the lists and sum - when a column is removed with the minus button, and then added again with the plus button, the previous content of the list, the name and the sum has to be restored before it was removed - the HTML webpage has to be designed in a clean way, with separate CSS, script and HTML, to make easier later to change the look of it, and you must not use any images. This means 3 files: [login to view URL], [login to view URL] and [login to view URL] (and additional files, if you use a JavaScript library) - it has to work on Windows, Mac and Linux, with Internet Explorer, Safari, Chrome and Firefox, and on iPhone, iPad and Android pads and phones. You can use any JavaScript library you want in addition to your JavaScript script, if the library is free to use for commercial applications, and if it works for all required systems and browsers, but I guess the application is so simple that you don't need any fancy library and can do most of it with a simple script. All files, including the library files, have to be provided locally, so that the application works offline, when I open it in a browser on my desktop. Delivery is one zip file with all files. But it also has to work when I deploy the files on a webserver and open it from my smartphones and pads.
