Convert html to fillable pdf form

The goal of the program is fill in data from a secure website and have the data output into PDF Forms that can be changeable after that. What I would like to do with the PDF Form Template that I have created in Livecycle is load it in the new program and have it recognize the fields from the user input and ? match it with those fields. I should also be able to edit the fields in the html to my liking but still be binded to the fields. This can happen one of two ways of binding, auto, or my manual input. I am also looking for the data to be stored online and have the forms changeable if necessary later on. There will be a few queries at first. The program must be able to choose the correct files from three different states and then be able to determine which files to use by whether it is a buyer or a seller. I have preliminary site already being constructed. I would like the data to be stored online via mysql or mssql or access The file should saveable by clicking File(or save) and it will process the form. How I would like them File or Saved would be by choosing a Root folder and then to have the capability to make specify "$folder/$folder/$filename" with field names from the form. So that way if i want to file them by client/project or if I wanted to file it by year/month. ## Deliverables Workflo 1 log into website must be secure 2 Choose the state they are working with IA, NE , SD (3? different acrobat forms) 3 Choose the section of forms to be used Buyer or Seller (another set of different acrobat forms) 4 Once this logic has been sorted user would fill in about 40 fields of questions. 5 Once the questions have been answered user presses view resultsor reset. 6 From these questions a number of pdf forms will be displayed, user can choose to open one or all to view? in acrbat reader? and then print and/or save the form to their computer. Documents to be saved by filename then property address last name 7 User will also be able to view all the documents they have built for each individual person by name, date, state and address 8 All data will be stored on the server and requires and admin and entry level user requirements 9 I would then like the data to be send as an email V-card to microsoft outlook. Note the pdf forms have all been made(in both livecycle and acrobat pro 9, with specific font and size and this will need to be followed throughout the project. I have drawn out how all the fields will match.
