jackson county list

This is list of properties. The .pdf was made by scanning a paper document. I need information from all pages into a single Excell document. I want the columns to be labeled: 1) PPIN, 2) Parcel, 3) Name, 4) Sold To, and 5) Rec No. Additional header information at the top of each page can be disreaguarded, as can any hand-written notes. I also want two more columns showing: 6) the page number each entry was on original document, and 7) the original, numerical order of each entry. I have attached 1) the .pdf that needs to be converted, and 2) an example of what I did with the spreadsheet from last year (you'll notice I added some extra columns once I started working with document)
