Website page design - 1 Landing page for a campaign

We are doing a prelaunch campaign for a new app (more info about the app will be disclosed later), we would like to do this campaign on Facebook and Instagram. The campaign will be an online contest where we will let the public to suggest a name for this app. We will be offering an iPhone X as a prize for the winner of the best name.
What we need:
- A landing page which is the form for the submissions. The form is basic which has "Name, Email, Phone and suggested names" fields. The page will have text which is about the app (no screenshots will be posted, we want to keep it secret), about the contest and T&Cs. The prize should be highlighted as well.
- Page should be responsive.
- Text will be provided.
- Webpages have to be responsive.
- No backend development needed, only frontend.
We need the design by today (February 20) (maximum tomorrow morning)
In order to select the best candidate, you would be required to send a wireframe/sample design of the page, more details will be sent when you apply.
