AngularJS application -- 2

I have a mockup of a web app designed in Indesign. I'm looking for an experienced developer that is able to take the Indesign mockup and generate an Angular or Ionic web app. The app will let the user enter personal data (name, surename, address, birth date, ecc) and select a day and a venue for an appointment. The web app can be previewed here: http://........ there is also a desktop mockup on how it should looks like on MD or LG resolutions: http://........ Is acceptable that the two versions (mobile/desktop) are on different HTML files. Webservices will be provided (RESTfull api that returns Json, no authentication)
