Monday.com Form - add some conditions and automation - Quick one to do now

We have a existing form with some hidden and some shown fields and also some conditions. We seek to update it with: 1. Make the date picker/calender able to be multiple dates selected eg: select Oct 3, OCT 6 and Oct 9 - right now only one date with selection tool. 2. Where it is currently 2 questions (1."Invoice number for this transaction" and "upload invoice" - both should be in the same question. "Add your invoice number below or upload your invoice if you prefer" 3.
