PHP Programmer needed for updates/corrections

I need a PHP programmer to help fix a few things and change a few others. This should be rather basic. Please see the attached document for the specifics. I have listed the fixes/changes in priority from most important to least important.
If you cannot complete the entire project within the budget, please specify a portion of the budget that you can complete for the bid amount, and I will consider separate GAF jobs for this amount. It is okay to separate this work into several projects, but please tell me if this is what you want to do.
I will escrow full amount once you are awarded the project, and I will pay immediately upon delivery of the changes. Communication is extremely important to me. I expect you to respond to emails and/or PMB posts.
Do NOT bid if you do not accept payment through GAF. Do NOT bid if you cannot complete this project in 3 days. Preference will be given to those with positive feedback and links to portfolio or samples of previous work.
I have many clients with frequent, ongoing projects. If you want regular, easy work like the above, you will have regular work with me. One of my current workers in India receives $150 from me each week for similar work.
